medication Under reviewAdenosine is a purine nucleoside that plays central roles in cellular energy transfer, signaling, and regulation of blood flow and neuronal activity. In cells, adenosine is a building block of adenosine triphosphate (ATP), the primary energy currency, and of nucleic acids. In the nervous system, adenosine acts as a neuromodulator with generally inhibitory effects, influencing sleepβwake regulation, fatigue, and vascular tone. Most commercial "adenosine" supplements are actually oral adenosine-5β²-triphosphate (ATP) rather than the nucleoside itself. Oral ATP is marketed to support muscular performance, strength, and recovery, particularly in resistance-trained individuals. Although intact ATP is not orally bioavailable, ATP and its breakdown products are metabolized in the gut and liver and may indirectly influence peripheral signaling pathways, blood flow, or muscle adaptation. Clinical studies in healthy resistance-trained men have examined daily oral ATP in the range of about 400 mg during structured resistance training programs. Beyond sports performance, adenosine as an injectable or intravenous drug is widely used in hospitals for acute cardiac indications, such as terminating certain supraventricular tachyarrhythmias and as a vasodilator in cardiac stress testing. These medical uses involve short-term, high-potency pharmacologic administration under supervision and are distinct from lower-dose over-the-counter oral ATP supplements intended for exercise performance.
Research summary
Human research on oral adenosine-5β²-triphosphate supplementation focuses primarily on resistance-trained healthy adults. Multiple randomized controlled trials and a recent meta-analysis indicate that daily oral ATP around 400 mg, combined with structured resistance training, can modestly increase gains in maximal strength, muscle mass, and power compared with placebo in healthy men. These studies also report good short-term safety, with no clinically significant changes in standard blood chemistry or hematology at doses up to several grams per day in the short term. However, mechanistic work in healthy humans shows that a single oral ATP dose does not measurably increase circulating ATP levels, suggesting poor direct bioavailability. The observed ergogenic effects are therefore likely mediated by indirect mechanisms, potentially involving metabolite signaling or local vascular changes. Evidence for benefits outside of resistance training, such as general energy enhancement or pain relief, is limited or absent in healthy humans, and the overall research base is still relatively small. Adenosine as an acute injectable or intravenous drug is well characterized for cardiovascular applications, but these data do not directly translate to chronic oral supplementation in healthy individuals.
