In 668 adults with moderate or severe migraine, single oral doses of almotriptan 12.5 mg and 25 mg produced significantly higher 2‑hour pain relief rates than placebo and were equivalent in efficacy to sumatriptan 100 mg, with almotriptan 12.5 mg as well tolerated as placebo and better tolerated than sumatriptan.

This narrative review summarizes randomized trials in over 4800 adults with moderate or severe migraine, concluding that almotriptan 12.5 mg is more effective than placebo and comparable in efficacy to sumatriptan 50 mg, with maintained response and favorable tolerability over up to one year of repeated use.

Across phase 2 and 3 double‑blind trials including more than 3500 migraine patients and long‑term open‑label studies in 1500 patients, almotriptan 12.5 mg consistently produced superior pain relief and pain‑free rates versus placebo with early onset of action and a lower incidence of adverse events compared with higher‑dose sumatriptan.

A systematic review of eight randomized controlled trials including 4995 participants found that almotriptan 12.5 mg was significantly more effective than placebo across all efficacy outcomes and had fewer adverse events than sumatriptan 100 mg, with slightly less efficacy than 25 mg almotriptan for very early pain‑free response.
