Research summary
Research in healthy human volunteers shows that aminophylline reliably increases ventilation, tidal volume, and ventilatory responsiveness to carbon dioxide, and can enhance the performance of fatigued respiratory muscles, particularly the diaphragm. Several small randomized or crossover studies in normal subjects demonstrate improvements in diaphragmatic contractility after induced fatigue and increased neural drive to inspiratory muscles, while therapeutic doses have minimal effect on contractility of a fresh, non-fatigued diaphragm. These same studies document dose-related activation of the sympathetic nervous system, with increases in catecholamines, heart rate, blood pressure, and metabolic markers, as well as a propensity for cardiac arrhythmias at clinically relevant concentrations. The overall research consensus is that aminophylline is effective as a bronchodilator and respiratory stimulant but carries a significant risk profile, making it unsuitable as a performance or wellness supplement for healthy individuals outside controlled medical settings.
