Research summary
Human studies of intranasal metoclopramide have focused on pharmacokinetics in healthy volunteers and on efficacy and safety in patients with diabetic gastroparesis, chemotherapy‑induced emesis, functional dyspepsia, and postoperative nausea and vomiting. Pharmacokinetic work in healthy adults shows that single intranasal doses achieve systemic exposure comparable to or somewhat lower than oral metoclopramide, with bioavailability around half that of oral dosing and onset of measurable plasma levels within approximately two hours. More recent development studies comparing different nasal doses to oral tablets in healthy volunteers have characterized dose–exposure relationships and supported selection of clinical dosing regimens. Clinical efficacy trials in patient populations consistently demonstrate that intranasal metoclopramide provides symptom relief for diabetic gastroparesis and is broadly comparable to oral metoclopramide for chemotherapy‑induced delayed emesis and functional dyspepsia, with some evidence of improved symptom control in gastroparesis relative to tablets. Across studies, the intranasal formulation is generally well tolerated, with the most common adverse effects being unpleasant taste, mild nasal irritation, headache, fatigue, and dysgeusia. As with all metoclopramide products, longer‑term or higher‑dose use carries a risk of central nervous system adverse effects, including extrapyramidal symptoms and tardive dyskinesia, so treatment duration is typically limited.
