supplement Under reviewResveratrol is a polyphenolic compound found in grapes, red wine, peanuts, and certain berries that has attracted intense interest as a potential longevity and cardiometabolic health supplement. It is often discussed together with other putative sirtuin-activating compounds (STACs) such as piceatannol, quercetin, fisetin, and certain synthetic molecules that modulate the activity of SIRT1 and related NAD-dependent deacetylases. These compounds are thought to influence cellular stress responses, energy metabolism, inflammation, and mitochondrial function, and are frequently positioned within the broader class of "calorie restriction mimetics" and nutraceuticals aimed at healthy aging. Mechanistically, resveratrol can activate SIRT1 in vitro and in animal models, modulate AMP-activated protein kinase (AMPK), improve endothelial nitric oxide signaling, and influence oxidative stress pathways and inflammatory gene expression. In humans, oral resveratrol is rapidly metabolized and has low systemic bioavailability, so most clinical trials have used relatively high doses of purified trans-resveratrol to attempt to overcome this limitation. Other sirtuin-activating polyphenols such as quercetin are often combined with resveratrol in multi-ingredient formulas to target vascular inflammation and oxidative stress, though this makes it difficult to attribute effects to a single compound. Overall, these compounds are primarily investigated for potential benefits on vascular function, cardiometabolic risk markers, cognitive aging, and general longevity-related pathways in otherwise healthy adults. From a practical health perspective, resveratrol and related STACs are used by healthy individuals mainly for theoretical longevity support, cardiovascular risk reduction, and neuroprotection rather than for treatment of a specific diagnosed disease. Human trials in healthy subjects have examined their ability to modulate endothelial function, inflammatory adhesion molecules, blood lipids, antioxidant capacity, and sirtuin expression. While some short-term studies in healthy volunteers show favorable shifts in vascular and oxidative biomarkers, larger and longer-term data in healthy populations remain limited, and the magnitude of effect on hard clinical outcomes is uncertain.
Research summary
Human clinical research on resveratrol and other sirtuin-activating compounds has expanded substantially over the past two decades, with hundreds of trials conducted across many conditions. However, studies specifically in healthy human subjects are fewer and generally small, often focused on surrogate biomarkers such as endothelial adhesion molecules, lipid profiles, antioxidant status, or SIRT1 expression rather than clinical endpoints. In healthy adults, some randomized controlled trials report modest improvements in cardiovascular risk markers, vascular inflammation, and total antioxidant capacity, especially at doses of 250–500 mg/day over several weeks. Yet other well-controlled studies in healthy or non-obese postmenopausal women have failed to confirm significant metabolic or vascular benefits, highlighting inconsistent effects and potential dependence on baseline metabolic status. Overall, the research consensus is that resveratrol is safe and well tolerated at doses up to several grams per day, but its poor oral bioavailability and mixed clinical results limit confidence in robust benefits for healthy individuals. The most consistently observed effects in healthy subjects are small improvements in selected vascular and oxidative stress biomarkers, without clear evidence of meaningful benefits on cognition, mood, or long-term cardiometabolic disease risk. For other natural sirtuin-activating compounds, human data in healthy populations are even more preliminary. As a result, resveratrol and related STACs should be viewed as promising but still experimental longevity-oriented supplements for healthy adults, with modest and variable effects rather than established, high-impact interventions.
