Research summary
Clinical research on tolazamide consists primarily of intervention studies in adults with type 2 diabetes, including randomized comparisons with other sulfonylureas and with exogenous insulin. These trials consistently show that tolazamide effectively lowers fasting blood glucose and glycosylated hemoglobin and can provide glycemic control comparable to that achieved with insulin in appropriately selected patients with residual beta-cell function. Other work indicates that improved glycemic control with tolazamide therapy is associated with normalization of whole-body glucose utilization toward nondiabetic levels in people with type 2 diabetes. The evidence base focuses on disease treatment rather than enhancement of health in otherwise healthy individuals. There are no controlled trials using tolazamide as a metabolic aid in healthy humans, and all efficacy data relate to patients with type 2 diabetes or related dysglycemia. Safety data from clinical use and prescribing information highlight hypoglycemia as the primary risk, along with infrequent dermatologic or gastrointestinal adverse effects. In contemporary guidelines, tolazamide is considered an older agent and is generally not used for wellness or performance purposes in healthy populations.
